Medical Documents for “J.M. Carras” Kindergarten

 

For student enrollment in “J.M. Carras” Kindergarten, parents must submit their child’s Personal Student Health Card (PSHC), completed by the child’s attending pediatrician (or general physician). Personal Health Cards signed by physicians of other specialties will NOT be accepted. The PSHC is valid for the whole period of the student’s attendance at Kindergarten (pre-K-Kindergarten). If there is any change in the student’s health status or risk factors after filling the PSHC, parents are required to immediately submit an updated one to “John M. Carras” Kindergarten.

Upon first-time enrollment of students in Kindergarten, in accordance with PD 79/2017, proof that the child has gotten all prescribed vaccinations must be demonstrated either by submitting the Personal Student Health Card or any other relevant document. If all prescribed vaccinations are not up to date, they should be completed on a timely basis so as not to interfere with enrollment.

Also upon the first-time enrollment of students in our Kindergarten, it is recommended that a preventive check of the student's vision (eye examination) and hearing (audiogram) be carried out in order to ensure the uninterrupted attendance of the school program.

An additional prerequisite for the completion of the online registration of students at "J.M. Carras" Kindergarten is the submission of a Medical Certificate, which is valid for one (1) school year and is duly signed by the pediatrician who attends the student, after a complete clinical and dermatological examination. Completion of the Medical Certificate by a physician of another specialty is not acceptable.

Parents of children with chronic diseases, severe allergies, or other serious health issues must also submit an additional certificate from the child’s attending physician outlining specific instructions on how the School should deal with an in-school emergency related to said disease, allergy, or other serious health issues.

 

Medical Documents for enrolment in “J.M. Carras” Kindergarten:

The forms can be obtained from the School’s Secretariat or can be downloaded and printed from the relevant links. The Medical Certificate must be submitted during the online registration and is a prerequisite for its completion.

ATTENTION

If the required medical assessment documents are not submitted by the start of the school year, the student will not be permitted to actively participate in Physical Education and swimming classes.


Medical Documents for HAEF Elementary Schools

 

For enrollment in Elementary School Grades 1 and 4, as well as for first time enrollment in any in-between grade, parents must submit their child’s Personal Student Health Card (PSHC) completed by the student’s attending pediatrician (or general physician). Personal Health Cards signed by physicians of other specialties will NOT be accepted. PSHCs are valid for three (3) years (ref. Φ.6/1094/80261/Δ1/20-5-2015 Ministry of Education circular). In the event that any health issues or risk factors arise after the submittal of a student’s Health Card, parents are obliged to immediately submit a new, updated Health Card to the Elementary School.

As per PD 79/2017, article 7, par. 4, for enrollment in Elementary School Grade 1 as well as first-time enrollment in any other Elementary School grade, proof that the child has gotten all prescribed vaccinations must be demonstrated either by submitting the Personal Student Health Card or any other relevant document. The same Medical Certificate will also be used for the student to participate in the School Varsity Teams and to take part in school championships (Interschool Games, A.S.I.S, Prefectural Games - National School Championships etc.), if selected for this purpose by the Physical Education Department, as well as in any additional optional activity offered by HAEF (Special Programs, Exercise - Sports, College Conservatory, Foreign Language Center etc.), in which the student may be enrolled during the same school year.

As of Grade 5,  for students who participate in competitive divisions or competitions, it is also recommended that a Cardiac Screening Evaluation, completed and signed by a cardiologist only, be submitted at the time of enrollment for the new school year.

Medical Documents for HAEF Junior High, High Schools & IBDP

 

For enrollment in Junior High School 1 (Grade 7) and High School 1 (Grade 10), as well as for the first-time enrollment in any in between grade and the IBDP, a Personal Student Health Card (PSHC) is required. PSHCs are valid for three (3) years (ref. Φ.6/1094/80261/Δ1/20-5-2015 Ministry of Education circular). In the event that any health issues or risk factors arise after the submittal of a student’s Health Card, parents are obliged to immediately submit a new, updated Health Card to the respective school unit.

ATTENTION: The PSHC up to the age of sixteen (16) years is completed by the pediatrician (or general physician) who attends the child. For children over the age of sixteen (16) years, PSHCs are completed by a pathologist or cardiologist or general physician. Completion of the PSHC by a doctor of another specialty is NOT acceptable. Exceptionally, the PSHC shall be completed by a pediatrician for students older than sixteen (16) years if the student is being attended by a special pediatric unit for specific disease conditions or if the student's biological maturation corresponds to childhood.
