Medical Documents for “J.M. Carras” Kindergarten

 

For student enrollment in “J.M. Carras” Kindergarten, parents must submit their child’s Personal Student Health Card (ADYM), completed by the child’s attending pediatrician. Students for whom the School has not received a completed Health Card by the start of the academic year will not be permitted to participate in Physical Education classes. In the event that any health issues or risk factors arise after the submittal of a student’s Health Card, parents are obliged to immediately submit a new, updated Health Card to “J.M. Carras” Kindergarten.

 

In accordance with PD 79/2017, article 7, par. 4, upon first-time enrollment in Kindergarten, proof that the child has gotten all prescribed vaccinations must be demonstrated either by submitting the Child’s Health Booklet or other relevant, official document. If all prescribed vaccinations are not up to date, they should be completed on a timely basis so as not to interfere with enrollment.

 

Parents of children with chronic diseases, severe allergies, or other serious health issues must also submit an additional certificate from the child’s attending physician outlining specific instructions on how the School should deal with an in-school emergency related to said disease, allergy, or other serious health issues.

 

It is also recommended that the child undergo visual (ophthalmological examination) and auditory (audiogram) screening in order to ensure that he/she can easily follow the school curriculum.

ATTENTION

 

  1. Medical specialists who may complete and sign a student’s Personal Health Card (ADYM) are pediatricians or general practitioners – and in special instances, pathologists or cardiologists. Personal Health Cards signed by physicians of other specialties will NOT be accepted.

    Medical specialists who may complete and sign the Certificate of Dermatological Examination are dermatologists, pediatricians, or general practitioners.

     

  2. If the required medical assessment documents are not submitted by the start of the school year, the student will not be permitted to actively participate in Physical Education and/or Swimming classes.

Medical Documents for HAEF Elementary Schools

 

For enrollment in Elementary School grades 1 and 4, as well as for first time enrollment in any in-between grade, parents must submit their child’s Personal Student Health Card (ADYM) completed by the student’s attending pediatrician. ADYMs are valid for three (3) years (ref. Φ.6/1094/80261/Δ1/20-5-2015 Ministry of Education circular). Students for whom the School has not received a completed Health Card by the start of the academic year will not be permitted to participate in Physical Education classes. In the event that any health issues or risk factors arise after the submittal of a student’s Health Card, parents are obliged to immediately submit a new, updated Health Card to “J.M. Carras” Kindergarten.

In order for a student to participate in swimming classes, at the beginning of each academic year, parents must submit to their child’s respective school unit a Certificate of Dermatological Examination that is valid for one (1) year. If the Certificate is not submitted before the start of the school year, the student will be unable to actively participate in swimming classes.

Medical Documents to Participate in Varsity Teams

In order for a student to participate in the School’s Varsity Team and to take part in school tournaments (inter-school competitions, A.S.I.S., Prefectural competitions, PanHellenic school championships, etc.), a Medical Certificate affirming the student’s fitness to participate in all school sports is desirable. The Medical Certificate should be submitted upon enrollment on an annual basis as it is valid for one (1) school year.

 

Additionally, from Elementary School grade 5 onwards, a Cardiac Screening Evaluation is desirable for those students who participate in competitive sports or matches. The Cardiac Screening Evaluation should be submitted at the time of registration.

    For enrollment in Junior High School grade 7 and High School grade 10, parents must submit to their child’s respective Secretariat the student’s Personal Student Health Card (ADYM). ADYMs are valid for three (3) years. If the Health Card is not submitted by the start of the academic year, the student will not be permitted to participate in Physical Education classes. In the event that any health issues or risk factors arise during the course of those three years, parents are obliged to immediately submit a new, updated Health Card to their child’s respective Secretariat.

     

    Medical specialists who may complete and sign a student’s Personal Health Card (ADYM) are:

     

    • pediatricians for students up to the age of sixteen
    • pathologists or cardiologist for students over the age of sixteen
    • Exception: students being monitored by special pediatric units for disease-specific illnesses or in instances where the student’s biological maturity corresponds to childhood, then his/her pediatrician may sign the Personal Health Card.

       

      Medical documents for swimming classes
